What is Guide?
The Guide program is a free Medicare-supported service that provides care cooridination, caregiver support, and help with everyday needs to ensure those living with dementia are able to remain in their homes and communities.
We’re here to support you and your loved one — no pressure, no changes to any benifits, just free help.
What types of support does GUIDE provide?
- Caregiver education, training, and respite services.
- Confidential support groups led by experts to share, connect, and feel understood.
- Comprehensive assessments for individuals and caregivers.
- A care plan that fits each person’s goals.
- 24/7 helpline for ongoing support and life.
- Coordination with local services (meals, transportation, etc.)
via a dedicated Care Navigator - Medication support and safety checks
- Regular check-ins and ongoing monitoring
- Support for transitions and referrals to specialists
- Engaging webinars on real caregiver challenges, hosted by
experts who understand.
Who’s Eligible?
Must be diagnosed with dementia
Must have Medicare Parts A & B as primary payer
Must live at home or in the community (not nursing home)
Cannot be enrolled in hospice
